At the preliminary inquiry, it was determined that:

Fred Jones, owner of the Jones Pulp Mill, was negligent by not immediately alerting township authorities of the dyke breach. He was fined substantially.

Officials at the Quibell pumping station were charged for:

• not having proper training routines in place for all pumping station personnel
• not having a documented and tested operational continuity plan in place
